Dear Continuing Education Provider:

Thank you for your interest in applying for provider and/or program approval from the Louisiana State Board of Massage Therapy.

Licensed Massage Therapists in Louisiana can obtain valid CEU credits only through providers and programs approved by the Board. We appreciate your interest in providing this service to Louisiana LMTs. Board-approved CEU providers in turn serve the public interest by promoting and enhancing professional standards and competence.

This application packet includes the following:

a. A Continuing Education Provider Application Form.

b. A Continuing Education Program Application Form, and

c. Instruction Sheets for both forms.

Please note that only Board-approved Providers can offer Board-approved Programs. Thus, even if you intend to offer only one program at this time, you still must apply for both Provider and program approval from the Board. And, you must pay both fees. You can, however, submit both forms at the same time. Your applications for Provider and Program approval will be reviewed at the same time.

Also, we recommend that you immediately make a number of photocopies of the Program application form. You can use these photocopied forms to apply for approval for future programs. That way you won't have to take up your time, and ours, for requests for more Program approval forms.
